1. What we collect
- Information you submit: name, phone number, email, mailing address, and health-coverage details (e.g., coverage status, household size, age band).
- A TrustedForm certificate capturing your consent, session activity, IP address, and timestamp at the moment you submit.
- Technical data: IP address, browser/device type, pages visited, and cookies/tracking technologies.
2. How we use it
To deliver the service you requested (connecting you with licensed insurance agents), to verify and process your information, to respond to you, to comply with law, and to improve and secure our services.
3. How we share and sell it
If you submit your information as a consumer, your information may be sold, licensed, or shared with one or more licensed insurance agents or agencies, who may contact you about insurance products by phone, text, email, or other means. We may also share data with service providers (hosting, analytics, payment, consent-verification) and marketing partners who help deliver or measure our campaigns. We do not sell or share the personal information of consumers we know to be under 16.
4. Your choices and rights
- Do Not Sell or Share My Personal Information — opt out at any time by emailing contact@healthproleads.com with the subject “Do Not Sell My Info.”
- Revoke consent / opt out of contact — withdraw consent to be contacted at any time; we honor and propagate revocation across our systems within 10 business days, as required by current FCC rules.
- Access / delete — depending on your state (including under the California Consumer Privacy Act), you may request to know, correct, or delete your personal information. We cannot contractually waive these rights.
5. Retention, security, transfers
We retain personal data only as long as needed for the purposes above or as required by law; use reasonable safeguards to protect it; and may store or process it on servers outside your state.
